Request for Copy of Tax Return (Spanish Version)
Form 4506 (sp) is the Spanish version of Form 4506, Request for Copy of Tax Return. Its Spanish title is "Solicitud de Copia de la Declaración de Impuestos". The form and its instructions on pages 3 and 4 are in Spanish, and it points to www.irs.gov/form4506sp for more information.
The IRS About page says Form 4506 is used to request a copy of your tax return or to name a third party to receive it. This edition asks for the return as originally filed with the IRS, with all schedules, Forms W-2 and amended returns. It charges $30 for each return requested, and the instructions say processing can take up to 75 calendar days.
When a transcript is enough, the form suggests Get Transcript at www.irs.gov/espanol or 1-800-908-9946 for faster service. The English edition is Form 4506. You can fill out the PDF free in your browser on this page and download it without an account.

The name shown first on the return and its SSN, ITIN or EIN, and on a joint return the spouse's name and number.
Your current name and address, and the address on your last filed return if it is different.
Name, address and phone number of a third party who should receive the return by mail.
The return number, such as Form 1040, 1040-SP, 1120 or 941, with a box to request certified copies for court or administrative proceedings.
Eight spaces for year or period end dates, the $30 cost per return, the number of returns and the total, and a box to send any refund to the third party.
An attestation box, the taxpayer's phone number, and lines for the signature, printed name, title and the spouse's signature.
The labels are in Spanish. The items below give their English meaning.
Enter the name shown first on the return. Enter that person's SSN or ITIN, or the business EIN. For a Form 1040 that includes Schedule C, the instructions say to enter your SSN.
Enter your current address, including a P.O. box if you use one, and the address on your last return if it differs. If the IRS does not have the new address, the instructions say to file Form 8822 or 8822-B with this request.
Name the third party on line 5 if one should receive the copy. On line 6, enter one return number only. Another type of return needs another Form 4506 (sp).
Enter the date each year or period ends. The line asks for dd/mm/aaaa, while the instructions on page 3 say mm/dd/aaaa and give 12/31/2018 for a 2018 Form 1040. List each quarter for quarterly returns.
Multiply $30 by the number of returns. Make the check or money order payable to "United States Treasury" and write your SSN, ITIN or EIN and the English words "Form 4506 (sp) request" on it. Check line 9 to send any refund to the line 5 third party.
Check the attestation box, or the form is returned unprocessed. Sign and date it. The IRS has to receive it within 120 days of the signature date. If your name changed, also sign your current name.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
Mail the form and payment to the RAIVS Team address for the state you lived in, or where the business operated, when the return was filed: Austin, TX 73301; Kansas City, MO 64999; or Ogden, UT 84409.
One chart covers individual returns (the Form 1040 series) and one covers all other returns. For several years that fall under two addresses, use the address for the most recent return.
The fee is $30 for each return requested, and a request without full payment is rejected. If the IRS cannot find the return, the form says the fee is refunded.
The instructions say it can take up to 75 calendar days to process the request.
Copies of Forms 1040, 1040-A and 1040-EZ are generally available for 7 years from filing. Other returns may be available longer.
For a transcript instead of a copy, the form points to Get Transcript at www.irs.gov/espanol or 1-800-908-9946: tax return, tax account, record of account and wage and income transcripts, and a verification of non-filing letter.
