8822-B
Change of Address or Responsible Party - Business
Form 8822-B notifies the IRS of a change to a business mailing address, a business location or the identity of a responsible party. The About page says it is for businesses and other entities with an Employer Identification Number application on file.
The instructions say any entity with an EIN reports a change in its responsible party on lines 8 and 9 within 60 days of the change, and that entities changing their address or responsible party file the form whether or not they are engaged in a trade or business. They also say that without a current mailing address or responsible party on file, the entity may not receive a notice of deficiency or a notice of demand for tax, while penalties and interest continue to accrue on any tax deficiencies.
A home address change uses Form 8822 instead. You can fill out Form 8822-B free in your browser on this page and download the result without an account.

Enter the business name and EIN, then the old and new mailing addresses with any apartment, room or suite number. The instructions say a P.O. box is entered only if the post office does not deliver mail to the street address.
Enter the new business location if it changed. For a foreign address, the instructions say to follow the country's practice for the postal code and not to abbreviate the country name.
If mail goes to a third party such as an accountant or attorney, the instructions say to enter C/O followed by the third party's name and street address or P.O. box.
Enter the new responsible party's name and identifying number. The instructions point to Form SS-4 and its instructions for who can be a responsible party and which number to enter.
An officer, owner, general partner or LLC member manager, plan administrator, fiduciary or authorized representative signs. A representative attaches a copy of the power of attorney, which can be Form 2848.
