Short Form Request for Individual Tax Return Transcript (Spanish Version)
Form 4506-T-EZ (sp) is the Spanish version of Form 4506-T-EZ, Short Form Request for Individual Tax Return Transcript. Its Spanish title is "Formulario Abreviado de Solicitud de Transcripción de la Declaración de Impuestos Individual." The form, its notes and its instructions on pages 2 and 3 are written in Spanish. The English edition is Form 4506-T-EZ.
The form says individuals can use it to request a tax return transcript for the current year and the three prior years, showing most lines of the original return. It says the transcript does not show payments, penalty assessments or adjustments made to the original return. The IRS About page adds that the transcript partially masks personal information while financial data stays fully visible, which is why the form has a customer file number line.
This edition is dated June 2023. The PDF can be filled out free in the browser on this page and downloaded without an account.

The name shown first on the return and its Social Security number or ITIN, then the spouse's name and number for a joint return.
Line 3 is the current name and address. Line 4 is the previous address shown on the last return, if different.
An optional number of up to 10 digits that prints on the transcript.
The year or years of the return transcript. The form says most requests are processed within 10 business days.
An attestation checkbox, the taxpayer's phone number, and signature and date lines for the taxpayer and spouse.
Purpose of the form, the customer file number, the Where to File chart with addresses and fax numbers, line instructions and the privacy notice.
The instructions say to enter the EIN if the request is for a business return. Otherwise enter the first SSN or ITIN shown on the return. For a Form 1040 with Schedule C, enter the SSN.
Enter the current address on line 3, including a P.O. box if used. Enter the address from the last return on line 4 if it differs. If the two differ and the IRS has not been told of the change, the instructions point to Form 8822.
Enter up to 10 numeric characters, or leave it blank. The instructions say not to use an SSN; if an SSN or name is used, the transcript shows "9999999999" instead.
Enter each year requested, written like the form's example "2008".
Check the attestation box, then sign and date exactly as the name appears on the original return. The instructions say a taxpayer whose name changed also signs the current name.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
Mail or fax the form to the address for the state lived in when the return was filed. With more than one transcript and two different addresses, use the address for the most recent return.
