ACH Vendor/Miscellaneous Payment Enrollment
Form 3881 is the ACH Vendor/Miscellaneous Payment Enrollment form. The form says it is used for Automated Clearing House (ACH) payments with an addendum record that contains payment-related information processed through the Direct Deposit Program. Its page 2 instructions refer to the Internal Revenue Service establishing ACH payments, also referred to as Electronic Funds Transfers (EFTs).
The form has three sections: agency information about the federal program agency originating the payment, payee or company information, and financial institution information with the routing and account numbers. The form says recipients of these payments should bring this information to the attention of their financial institution when presenting the form for completion.
The Privacy Act statement says the Treasury Department uses the information to transmit payment data by electronic means to the vendor's financial institution. You can fill out Form 3881 free in your browser on this page and download the result without an account.
