Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites (Korean Version)
Form 15080 (ko) is the Korean version of Form 15080, Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites. The IRS publishes it for taxpayers who read Korean and have their return prepared at a VITA or TCE volunteer site. The consent text on the PDF is in Korean, under the title "VITA/TCE 세금 작성 사이트에 세금 신고 정보 공개 동의". The header is dated 2025년 10월 (October 2025) and the footer prints Form 15080 (ko) (Rev. 10-2025).
The form asks whether the taxpayer agrees to 전반적 정보 이월 (Global Carry Forward). The Korean text says this lets TaxSlayer LLC, the provider of the VITA/TCE tax software, make the return information available at every volunteer site in the IRS VITA/TCE program that the taxpayer chooses in the next filing period. This edition says the consent is valid until 2027년 11월 30일 (November 30, 2027).
The English edition, Form 15080, has its own page. You can fill out this Korean PDF free in your browser on this page and download the result without an account.

The form says a taxpayer who wants an earlier end date or a narrower scope will refuse consent instead of signing.
To consent, the primary taxpayer signs by hand and enters the date (날짜) on the first line.
The PDF prints a second signature and date line. In this edition it carries the same label as the first line.
The Korean text says the taxpayer has the right to receive a signed copy of the form.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form is signed at the site that prepares the return. It prints no mailing address.
The form says it is not needed to use the site's services, and a signature obtained as a condition of those services makes the consent invalid.
The terms say the consent is valid until 2027년 11월 30일. The federal disclosure adds that a consent with no period stated is valid for one year from the date of signature.
Consenting allows the preparer to enter a PIN in the tax software on the taxpayer's behalf to confirm agreement with the terms.
The form says to call TIGTA at 1-800-366-4484 if return information was disclosed or used improperly or without permission.
