Form 13614-C (zh-s) in Simplified Chinese: Intake/Interview and Quality Review Sheet

Form 13614-C (zh-s) is the Simplified Chinese version of Form 13614-C, Intake/Interview and Quality Review Sheet. The IRS PDF is written in Simplified Chinese for taxpayers who read Chinese and get help at a Volunteer Income Tax Assistance (VITA) or Tax Counseling for the Elderly (TCE) site. The English edition is Form 13614-C. This page describes the Simplified Chinese edition in English.

This edition is dated October 2026 and its questions ask about 2026. It asks the taxpayer to complete pages 1 to 4 and to bring tax information such as Forms W-2, 1099, 1098 and 1095, Social Security cards or ITIN letters for everyone on the return, and photo ID such as a valid driver's license for the taxpayer and spouse. Columns marked to be completed by the certified volunteer are for the IRS-certified volunteer preparer. The PDF also includes Form 15080 (zh-s), a consent to disclose tax return information to VITA/TCE sites.

The PDF can be filled out free in the browser on this page and downloaded without an account.
