Insurance Deduction Authorization
VA Form 29-888 is the U.S. Department of Veterans Affairs Insurance Deduction Authorization, for deduction from benefit payments. VA's form page lists it under life insurance. With it, the insured authorizes VA to deduct each month from benefits payable the sum shown on the form for premiums and the repayment of loans and/or liens, and to adjust the amount within the limits of benefits payable to pay premiums on Government Life Insurance. The form says that unless the insured specifies otherwise, the authorization covers all Government Life Insurance policies under the insured's ownership.
The PDF has two pages. Page 1 holds Items 1 to 10, from the insured's name and policy number to the action requested, the deduction amounts and the signature. Page 2 holds important information and instructions on how the deductions work and where to send the form.
