VA Form 21P-524: Statement of Person Claiming to Have Stood in Relation of Parent

VA Form 21P-524 is the U.S. Department of Veterans Affairs Statement of Person Claiming to Have Stood in Relation of Parent. A claimant uses it to describe the relationship they had with a deceased Veteran: when and how they got custody or care of the Veteran, who supported the Veteran, and other facts that show the relationship. The form's burden notice says VA needs the information to determine eligibility for service-connected death benefits (38 U.S.C. 1315 and 5101).

The form is six pages. Part I is the claimant's own statement, and Parts II and III are statements from two disinterested persons who have personal knowledge of the relationship between the claimant and the Veteran. Each part ends with a certification and a signature in ink.

VA's page lists the form under family member and caregiver benefits. You can fill out the PDF free in your browser on this page and download the result without an account.
