VA Form 21P-535: Application for Dependency and Indemnity Compensation by Parent(s)

VA Form 21P-535 is the U.S. Department of Veterans Affairs Application for Dependency and Indemnity Compensation by Parent(s) (Including Accrued Benefits and Death Compensation When Applicable). Its instructions say to use it to apply for VA benefits as the surviving parent(s) of a deceased veteran, and for any money VA owed the veteran but did not pay before death, called accrued benefits.

VA's page says a surviving parent fills out this form. A surviving spouse or child of a Veteran uses VA Form 21P-534EZ, and for a service member who died on active duty, a military casualty assistance officer helps the spouse or child complete VA Form 21P-534a.

The PDF has 10 pages: two pages of general instructions, the application in Sections I to XI, and Form SSA-24, an optional Application for Survivors Benefits that VA sends on to the Social Security Administration. You can fill out the PDF free in your browser on this page and download the result without an account.

How to apply with VA Form 21P-535

  1. 1

    Read the general instructions

Print or type all answers clearly. The instructions say to write "none" or "0" where that is the answer, and "unknown" if you do not know.

  2. 2

Gather the documents it asks for

The form asks for a copy of the death certificate, with listed exceptions, the original DD214 or a certified copy if the Veteran never filed a VA claim, and the Veteran's public record of birth or court record of adoption.

  3. 3

Report income as the form says

Report totals before deductions, do not report the same income in both tables, and include your most recent award letter if you receive monthly benefits. The form says a blank space means "0" or "None".

  4. 4

    Decide on Form SSA-24

Mail it to the Pension Intake Center, upload it with QuickSubmit, or get help from an accredited representative or a VA regional office. The instructions say to make a photocopy of everything you send.
