Priority Processing Request (VA Form 20-10207)
VA Form 20-10207 is the U.S. Department of Veterans Affairs Priority Processing Request. VA's form page says to use it to request priority processing for your claim due to certain qualifying circumstances or status, and the page heading calls it the priority processing request form for an existing claim.
The instructions list circumstances and the evidence to send with each, if available or not already on file with VA: extreme financial hardship, a terminal illness, an ALS diagnosis, being very seriously or seriously injured or ill during military operations, age 85 or older, former prisoner of war status, and Medal of Honor or Purple Heart recipients. The form also asks about homelessness or the risk of becoming homeless.
The PDF is five pages: two pages of instructions and mailing addresses, then Sections I to VII. VA's page says the request can also be submitted online. You can fill out the PDF free in your browser on this page and download the result without an account.

Items 1 to 8: the Veteran's name, Social Security number, date of birth, VA file number and insurance number if applicable, mailing address, phone and email.

Items 9 to 15 repeat the identification fields for a claimant other than the Veteran.
Whether you are currently homeless or at risk of becoming homeless, which statements describe your living situation, and a point of contact with a phone number. The form says VA uses this information only to prioritize the request.
Check boxes for extreme financial hardship, terminally ill, ALS, very seriously or seriously injured or ill during military service, former prisoner of war with dates of confinement, Medal of Honor or Purple Heart recipient, and 85 years of age or older.

Item 18 lists VA medical centers, DoD military treatment facilities or private facilities where you were treated for the circumstance in Item 17, with the approximate start date of treatment.
The requester's certification and signature, marked required, then signature sections for a third party and for a Power of Attorney.
Pages 1 and 2 list the evidence for each circumstance and the mailing addresses. The form says to read the Privacy Act and Respondent Burden on page 5 before completing it.
Identify the Veteran and any other claimant, then mark the reasons. If completing by hand, the form asks for neat, legible printing in ink and completely filled check boxes.
The instructions name items such as an eviction notice or statement of foreclosure, medical evidence or a DD Form 214, and VA Forms 21-4142 and 21-4142a if you want VA to get private treatment records.
Sign and date Item 19. A third party or Power of Attorney signs Section VI or VII, with the authorization form the section names.
The form says the fastest way to respond to VA is 1-800-827-1000. To mail it, identify the benefit type and use the matching address on page 2, or submit the request online through VA's page.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
