Document Evidence Submission
VA Form 20-10208 is the U.S. Department of Veterans Affairs Document Evidence Submission. The form says it is used for the submission of additional documentation or evidence. It identifies the Veteran or service member, and the claimant when that is someone else, lists the types of documents or evidence being sent, and carries a certification and signature.
The PDF is two pages with five sections. Section III asks whether the form is sent in response to a request from VA and has check boxes for document types such as birth, marriage and death certificates, medical treatment records and service treatment records. For a written statement, the form points to VA Form 21-10210, Lay/Witness Statement, instead. You can fill out the PDF free in your browser on this page and download the result without an account.
