VA Form 10-306: Request for Information (PCAFC Decisions)

VA Form 10-306 is the U.S. Department of Veterans Affairs Request for Information about Program of Comprehensive Assistance for Family Caregivers (PCAFC) Decisions. The form says it is used to request information about decisions made related to an application for or participation in PCAFC, and that this information may help in deciding whether to file a Notice of Disagreement (NOD) or otherwise request VA's review of a decision.

The form says VA searches the Caregiver Application Record Management Application (CARMA) and, for every VA Form 10-10CG application it has received from the requester, provides decisions, dates of decisions and copies of decision letters VA issued, to the extent available. VA also searches the Patient Advocate Tracking System-Replacement (PATS-R) tool for appeals, and letters from the Office of Community Care's Caregiver Stipend Program.

The form is two pages: instructions, then Section I for the individual requesting information and Section II for the signature. You can fill out the PDF free in your browser on this page and download the result without an account.

Who uses VA Form 10-306?

  • Veterans and caregivers in PCAFC

The form says it is for Veterans and caregivers who have applied for or participated in PCAFC and would like information regarding one or more previous PCAFC decisions.

  • Primary and Secondary Family Caregivers

A VA Form 10-10CG application can name one Primary Family Caregiver and up to two Secondary Family Caregiver applicants. Section I asks whether you applied as a Veteran, Family Caregiver or both at different times.

  • One person per form

VA releases information only about the individual named in Section I. The form says each person requesting information completes a separate VA Form 10-306 about themselves.

  • What a Veteran's request covers

The form's example: a Veteran's request covers the names of co-applicants, VA's decisions and dates, the approved stipend tier or denial reason, and any discharges or revocations, but not decision letters sent to the caregivers.

The form describes who completes it and whose information VA releases.
