VA Child Care Subsidy Program Benefit Payment Request Form
VA Form VA0730h is the U.S. Department of Veterans Affairs VA Child Care Subsidy Program Benefit Payment Request Form. The instructions say employees approved to participate in the VA Child Care Subsidy Program (CCSP) must use this form to request monthly child care subsidy benefits payments.
It is a VA employee form, not a public one. The employee fills in their details, the child care provider's details and the weekly costs for one month, then signs. Section IV is for approving office use only. Requests go through the Child Care Records Management System (CCRMS).
The form is one page. You can fill out the PDF free in your browser on this page and download the result without an account.

When and how to submit, what the provider's invoice should include, and a note that the monthly subsidy is forwarded to the child care provider on the employee's behalf.

Name and home address of the employee, home phone, home email if applicable, work address, work phone, station number and work email if applicable.

The provider or company's name and address, telephone and email, the children's names and ages, the month and year requested, and the full child care cost for each week of that month.

The employee certifies that the information is true and correct and that the child care services were received for the period the request covers, then signs and dates it.

Monthly cost, VA percentage, subsidy amount, a disapproved box with an explanation, the authorizing official's signature and date, a comment, the date paid and the invoice number.
The instructions say a separate form is prepared for each month and is not submitted until child care services for the month have been provided.
Enter your details, the provider's details and each child's name and age.
Item 5 is the month and year requested. Item 6 asks for the full child care cost for each week, with week ending dates, weekly total charges and a monthly total.
Sign and enter the date of signature as MM/DD/YYYY. Leave Section IV for the approving office.
The instructions say to check that the CCRMS online request matches the invoices, and to submit through CCRMS not later than the second Friday of the month following the month requested.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form says benefit payment requests are submitted through the CCRMS, and "Mailed, faxed or emailed submissions will not be accepted."
The form says requests are due not later than the second Friday of the month following the month for which a benefit payment is requested. Its example: for June, the invoice is received not later than the second Friday of July of the same year.
The form says invoices that are not received within the required timeframe will not be paid, and exceptions to this timeframe will not be approved for any reason.
