Its Privacy Act statement says Public Law 107-67 confers regulatory authority on VA for agency use of appropriated funds for child care costs for lower income Federal employees, and that the information is used to determine eligibility for child care subsidy.
It is a VA employee form. The parent or legal guardian gives their job series and grade and a VA organizational code, and the form refers to the submitting HR office and the local Human Resources (HR) office.
The form is two pages: Section I for the parent or legal guardian, Section II for up to three children, and Section III for the signature and certification. You can fill out the PDF free in your browser on this page and download the result without an account.
