Child Care Provider Information (For the Child Care Subsidy Program)
VA Form VA0730b is the U.S. Department of Veterans Affairs Child Care Provider Information form for the VA Child Care Subsidy Program. Its Privacy Act statement says the provider's Social Security or tax identification numbers are used to assure licensure and/or regulation compliance, which is necessary for determining Federal employee eligibility for child care subsidy.
The form is for VA employees in the program and their child care providers, not the general public. Part I names the parent, with the parent's Federal agency printed as Department of Veterans Affairs, and Part IV is the provider's certification and signature.
The form is one page. You can fill out the PDF free in your browser on this page and download the result without an account.

Part I takes the name of the parent or legal guardian. The Federal agency is already printed as Department of Veterans Affairs.
