Supplemental Questionnaire for Selected Positions
Standard Form 85P-S (SF 85P-S) is the Supplemental Questionnaire for Selected Positions. It is printed as a U.S. Office of Personnel Management (OPM) form, Revised December 2017, under 5 CFR Parts 731, 732, and 736, and its header prints OMB No. 3206 0258. GSA's forms library hosts a copy.
The form says it is supplemental to SF 85P, Questionnaire for Public Trust Positions, but is used only after an offer of employment has been made and when the information it requests is job-related and justified by business necessity. Otherwise it has the same purposes and authorities described on SF 85P, and the instructions for completing it are the same as SF 85P.
SF 85P-S has five sections: full name, Social Security number, use of illegal drugs and drug activity, use of alcohol, and psychological and emotional health, followed by a signed certification. The form says the agency which gave it to you will tell you which questions to answer. You can fill it out free in your browser on this page and download the result without an account.

Your full name exactly as it appears on your SF 85P, and your Social Security number.

Whether, since the age of 16 or in the last 5 years, whichever is shorter, you illegally used any controlled substance or prescription drugs, and whether you ever did so while employed as a law enforcement officer, prosecutor or courtroom official, while possessing a security clearance, or in a position directly and immediately affecting the public safety.

Whether, in the last 5 years, your use of alcoholic beverages resulted in alcohol-related treatment or counseling, with the dates and the name and address of the counselor or doctor.
Whether a court or administrative agency EVER declared you mentally incompetent or ordered you to consult a mental health professional, and whether you were EVER hospitalized for a mental health condition.
Whether you were EVER diagnosed with psychotic disorder, schizophrenia, schizoaffective disorder, delusional disorder, bipolar mood disorder, borderline personality disorder or antisocial personality disorder, and any current treatment.
A question on any mental health or other health condition that substantially adversely affects judgment, reliability or trustworthiness, then the signed certification.
The form says the agency which gave it to you will tell you which questions to answer, and that the instructions for completing it are the same as SF 85P.
Enter your full name exactly as it appears on your SF 85P, then your Social Security number.
Each question has YES and NO boxes. A Yes opens entry blocks for dates, names and addresses, and many questions say where to proceed on No.
The form says to complete 5E only if you responded No to all of 5A, 5B, 5C and 5D. If Yes was selected for any of them, it says to proceed to the certification.
Sign in ink and date the Certification That My Answers Are True on the last page.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no filing address. The burden notice says not to send your completed form to the OPM address printed there.
OPM estimates the public burden at an average of 10 minutes per response, including time for reviewing instructions and gathering the data.
The form says mental health treatment and counseling, in and of itself, is not a reason to revoke or deny eligibility, and that seeking or receiving care may contribute favorably to decisions about your eligibility.
Section 3 says neither your truthful responses nor information derived from them will be used as evidence against you in a subsequent criminal proceeding.
The certification says a knowing and willful false statement on this form can be punished by fine or imprisonment or both, citing section 1001 of title 18, United States Code.
It supplements SF 85P, Questionnaire for Public Trust Positions. Section 1 asks for your full name exactly as it appears on your SF 85P.
The form says it is used only after an offer of employment has been made, and only when the information it requests is job-related and justified by business necessity.

Its note says that if your judgment, reliability or trustworthiness is not substantially adversely affected, answer no even if you have a condition requiring treatment, with examples such as service as a first responder or in a military combat environment.
