Agency Request to Pass Over a Preference Eligible or Object to an Eligible
Standard Form 62 (SF 62), Agency Request to Pass Over a Preference Eligible or Object to an Eligible, is a U.S. Office of Personnel Management (OPM) form, Revised April 2011. GSA hosts a copy in its forms library, and GSA's page lists the authority as 5 CFR Parts 332 and 338.
The form says it is used to object to an eligible or to request authority to pass over a preference eligible and appoint a nonpreference eligible. It is completed and signed by the agency appointing official, not by the eligible. Part A covers qualifications or conduct reasons. Part B is restricted to medical decisions resulting in a pass over of a preference eligible. Each part ends with a decision block: Part A for an agency official or OPM official, Part B for an OPM official.
The form says to submit it in duplicate with the pertinent documents, and one copy is returned with the decision. You can fill it out free in your browser on this page and download the result without an account.

A space to state the reasons specifically and clearly, then the signature and title of the agency appointing official, telephone number and date.
Boxes for whether the action is sustained and the eligible is removed from consideration, or not sustained with reasons, and the deciding official's signature and date.
For medical pass overs of a preference eligible: the same items 1 to 9, the action proposed, and medical reasons as they relate to the physical requirements and environmental conditions of the position.
Boxes for action sustained or not sustained, a Remarks space, and the signature, title and date of the OPM official.
Part A is for qualifications or conduct reasons. The form says use of Part B is restricted to medical decisions resulting in a pass over of a preference eligible.
Enter the agency contact, the eligible's name and address, the rating, the certificate number and date, the position title and grade, and the title of examination.
Check the objection or pass over box. For a veteran with a service-connected disability of 30 percent or more, the form says to attach a copy of the notification.
Part A says to state reasons specifically and clearly so that the significance is readily apparent. Part B says to briefly explain the decision as it relates to the physical requirements and environmental conditions of the position.
Part A lists the vacancy announcement, position description, application, certificate of eligibles and service record. Part B lists the position description, application or work history and pertinent, current medical evidence and prior reports.
The agency appointing official signs. The form says to submit it in duplicate, and one copy is returned with the decision.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF prints no mailing address. It says specific processing and mailing instructions are in the Delegated Examining Operations Handbook, Chapter 6, Section D, Object to an Eligible.
The form says that pending approval, the agency may not appoint an eligible who would be within reach only if the action is approved.
The veteran is notified of the action, the reasons and the right to respond to OPM within 15 days of the notice, under 5 U.S.C. 3318(b)(2) in Part A and 5 U.S.C. 3312(b) in Part B.
Part A cites 5 CFR 332.406 and its footer 5 CFR part 332 and 338. Part B cites 5 CFR 339 and its footer 5 CFR part 332 and 339.
The footer reads SF 62, Revised April 2011, and says all previous editions are not usable.
The agency appointing official states the reasons and signs with title, telephone number and date. Part B's instructions are addressed to the agency appointing official.
Items 1 to 3 hold the agency contact's name and mailing address, e-mail address and telephone number.
Part A ends with an OPM/Agency Decision signed by an agency official or OPM official. Part B ends with an OPM Decision signed by an OPM official, with remarks.
The form says eligibles have the right to request and review the reasons. A veteran with a service-connected disability of 30 percent or more is notified and may respond to OPM within 15 days of the notice.
