Standard Form 336 (SF 336): COOP Alternate Facility Identification/Certification

Standard Form 336 (SF 336), Continuity of Operations (COOP) Alternate Facility Identification/Certification, Rev. 6/2026, is a form the instructions say was designed to help Federal Departments and Agencies with identifying their continuity facilities. Its footer reads Required by Presidential Policy Directive 40 (PPD-40). The form names no prescribing agency. GSA hosts it in its forms library, where it is listed as Continuity of Operations (COOP) Continuity Facility Identification/Certification.

The form says PPD-40, National Continuity Policy, mandates Federal Departments and Agencies (D/As) to report reconstitution requirements to the U.S. General Services Administration (GSA) annually, and that the annual submission of SF 336 to GSA fulfills department and agency requirements for this directive. It covers agency information, the classification of the completed form, the primary facility, each alternate facility, points of contact and a certification, followed by an adjudication block completed by GSA.
