Standard Form 2050 (SF 2050): Reconstitution Questionnaire

Standard Form 2050 (SF 2050), Reconstitution Questionnaire, is a form federal departments and agencies use to document the reconstitution needs of their headquarters facilities. Its footer prints STANDARD FORM 2050 (REV. 4/2025) and Required by Presidential Policy Directive 40 (PPD-40). The form names no other prescribing agency, and GSA hosts it in its forms library.

The form says PPD-40, National Continuity Policy, mandates Federal Departments and Agencies (D/As) to report reconstitution requirements to the General Services Administration (GSA) annually. It is marked CUI When Filled In. Its 10 sections cover the current and projected facility, points of contact, office space and furniture, communications, warehouse and storage, HVAC and power, security, miscellaneous needs, attachments and a signed certification.

The form says submission does not obligate the government nor guarantee future funding. Its instructions start on page 8. You can fill it out free in your browser on this page and download the result without an account.

How to complete SF 2050

  1. 1

    Check for classified content first

The stipulations say that if the form will contain any classified information, contact [email protected] or call GSA at (202) 501-0012 before filling it out.

  2. 2

    Answer box 2j

No means completing the full questionnaire. Yes, All Needs skips to Section 8, number 18. Yes, Some Needs means listing only the needs not covered by the agency's other locations.

  3. 3

    Complete each box you reach

Unless box 2j is Yes, All Needs, the form says a minimum of the letter a should be completed in each later box, and to skip questions only where the questionnaire says to.

  4. 4

Measure space as the form defines it

SF means ANSI/BOMA Office Area (ABOA) square feet. The form says GSA recommends an industry standard of 200 square feet per full time employee when estimating future needs.

  5. 5

Attach extra sheets and list them

Several boxes allow additional sheets. Box 19a lists the attachment types to check, or None if there are no attachments.

  6. 6

    Date and sign the certification

Box 20a takes the month and year the information was last verified, which the form says can differ from the signing date. The signer then adds name, title, signature, email and phone.
