Standard Form 3116 (SF 3116): Phased Employment/Phased Retirement Status Elections

Standard Form 3116 (SF 3116), Phased Employment/Phased Retirement Status Elections, is a U.S. Office of Personnel Management (OPM) form dated October 2014, from the CSRS/FERS Handbook for Personnel and Payroll Office. GSA hosts a copy in its forms library.

The form says phased employment/phased retirement is not an employee right, but an option that requires the consent of both the employee and the employing agency. A phased retiree works part time at a working percentage, currently 50 percent, and receives a phased retirement annuity. The package holds information pages and four election parts: Part 1A to elect it, Part 2A to end it and return to regular employment status, Part 3A for a new agency's consent to continue it after a move, and Part 3B to end it upon a move to another agency.

You can fill it out free in your browser on this page and download the result without an account.
