Application for 10-Point Veteran Preference
Standard Form 15 (SF 15), Application for 10-Point Veteran Preference, is a U.S. Office of Personnel Management (OPM) form to be used by veterans and relatives of veterans. The form says the information is used, along with the documents sent with it, to determine whether the applicant is entitled to 10-point veterans' preference. GSA hosts a copy in its forms library.
The form lists five types of claim, based on the veteran's disability, Purple Heart or pension, or on preference for a veteran's spouse, widow or widower, or mother. Each type points to the documentation listed on page 2. The form notes that eligibility is governed by 5 U.S.C. 2108, 2108a and 5 CFR part 211, and that not all conditions are fully described on the form.
OPM estimates about 10 minutes per response. You can fill it out free in your browser on this page and download the result without an account.
