Schedule of Canceled or Undelivered Checks
Standard Form 1098 (SF 1098), Schedule of Canceled or Undelivered Checks, is a U.S. Department of the Treasury form. Its header prints Department of the Treasury, Revised July 1980 and I TFRM 4-7000, and GSA's page lists its authority as Treasury Financial Manual Volume 1, Part 4, Chapter 7000. GSA hosts a copy in its forms library.
Its blocks describe a federal department's office and accounts, not an individual filer: department or establishment, bureau or office, location, accounting period, schedule and sheet numbers, D.O. symbol number and agency location code (ALC). Boxes at the top mark the schedule as Canceled or Undelivered, and a table lists each check by date of issue, check number, payee, voucher number, amount and the appropriation or fund to be credited. A closing statement records that the amount of the checks was deposited for credit in the account of the U.S. Treasury on a deposit ticket.
The form prints no instructions. You can fill it out free in your browser on this page and download the result without an account.
