GSA Form 350: Inspection Report of Unfired Pressure Vessels

GSA Form 350, Inspection Report of Unfired Pressure Vessels, is a General Services Administration form. Its heading names the Public Buildings Service, and each page footer reads GSA FORM 350 with the edition date (3-51). GSA hosts it in its forms library.

The four-page report covers one vessel: the heading says to make a separate report for each pressure vessel. It records the building, region and date of inspection, the vessel's type, construction and manufacturer's rating, its condition, a pressure test, relief valves, gauges and other controls, and the inspector's opinion on whether the vessel is in a safe condition to be operated. Items marked with an asterisk are to be reported at first inspection only. The form prints no OMB number and no privacy statement.

You can fill it out free in your browser on this page and download the result without an account.
