GSA Form 1228, Monthly Report of Real Property Appraisal Contracts Awarded, is a General Services Administration form, GSA Form 1228 (Rev. 7-86). GSA hosts it in its forms library; GSA's page lists no authority or regulation. It carries the reports control symbol PB-107.
