Condition Survey Report
GSA 1204, Condition Survey Report, is a General Services Administration form, Rev. 9/2014. GSA hosts it in its forms library.
The form records the condition of premises under a lease. It names the building, the lessor, the room numbers or other identification and the lease number, and it is marked as an INITIAL or FINAL survey with a date of survey. A list of suggested items, which the form labels incomplete, runs from the building exterior to the windows, and a narrative report beside it comments on each pertinent item.
The undersigned certify that the report represents their opinion of the true and correct condition of the premises as of a stated date, and the lessor signs as well. You can fill it out free in your browser on this page and download the result without an account.
