GSA Form 1181: Cleaning Inspection Report

GSA Form 1181, Cleaning Inspection Report, is a General Services Administration (GSA) Public Buildings Service form, printed as GSA FORM 1181 FEB 64. GSA hosts it in its forms library, and GSA's page lists no authority or regulation for it.

The instructions on the back say the GSA Handbook of "Operation and Maintenance of Real Property" prescribes the use of this form to record on-site inspection of cleaning operations in GSA operated and in leased owner-serviced buildings.

The front lists services in categories A to I, items 1 to 48, and items 49 to 59 for condition or status, each rated on a scale of 1 to 5. The ratings add up to a total, and the form says the highest possible rating is 100. Conditions to be corrected are listed by item number with the date corrected and a signature. You can fill it out free in your browser on this page and download the result without an account.
