G-1041A
Genealogy Records Request
Form G-1041A, Genealogy Records Request, is the USCIS form for getting copies of USCIS historical records. The instructions say requests have to identify the record by its file number or another file identifier, because USCIS cannot retrieve a record without one.
Without a valid USCIS file number, the instructions point to Form G-1041, Genealogy Index Search Request, which searches the historical indices first. The record series on Form G-1041A are Naturalization Certificate Files (1906 to 1956), non-standard C-Files, Alien Registration Forms AR-2 (1940 to 1944), A-Files numbered below 8 million, Visa Files (1924 to 1944) and Registry Files (1929 to 1944).
The instructions say this office can only provide photocopies, not certified copies.

Your full name, mailing address, daytime and mobile phone numbers and email address, plus the requestor's certification and signature, where you consent to pay the costs of search, duplication and review.
Whether the file information came from an earlier Genealogy Index Search Request and its case number, the immigrant's full name and other names used, date of birth (actual or estimated) and place of birth.

One block per record series: Naturalization Certificate File, non-standard C-Files, Alien Registration Record (AR-2), A-File numbered below 8 million, Visa File and Registry File, each with its file number, details and a copy format box, then a total fees line.

Extra space for longer answers, with page, part and item number columns. The form says more copies of the page or a separate sheet can be attached.
The instructions say USCIS cannot retrieve a record without a file number. Without one, the starting point is Form G-1041, and the file numbers it returns are entered here, with the index search case number in Part 2.
The instructions have a chart of sample numbers, such as C-1234567 for a Naturalization Certificate File, AR-1234567 for an AR-2 record and R-12345 for a Registry File. They say not to give a Certificate of Registry number such as CR-12345.
Enter the immigrant's name, other names including aliases, maiden name and nicknames, and the date and place of birth. Mark whether the date of birth is actual or estimated.
For an immigrant born less than 100 years before the request, include a copy of one proof of death item. The instructions say not to send original records because USCIS will not return them.
Answer every question. Write N/A where a question does not apply and None for a zero. Extra answers go in Part 4 with your name and the page, part and item number.
The instructions say a stamped or typed name is not accepted and an unsigned request is rejected. A photocopy, fax or scan of an original handwritten signature counts. This page lets you fill the form online free, then print it for signature.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The USCIS page lists two options: online through the USCIS Genealogy Program page, or by mail with payment to the Chicago Lockbox, Attn: Genealogy Program. Check the form page for the current address before mailing.
The fee is on the USCIS Fee Schedule page (Form G-1055), which can change. Check it on the day you file. Part 3 has a copy format box for each record and a total fees line.
The USCIS page lists edition 04/01/24. It says all pages need to come from the same edition, with the edition date and page numbers visible at the bottom, or USCIS may reject the form.
Send legible photocopies unless an original is requested. A foreign-language document needs a full English translation with a certification from the translator.
The instructions say this office can only provide photocopies. A Certification of Nonexistence of a Naturalization Record is requested from the USCIS Records Operations Branch instead.
The instructions list a researcher requesting a copy of a USCIS historical record by file number, and a researcher who received file numbers from a Form G-1041 index search.
The instructions also list researchers seeking records for genealogical or family history purposes or heir location who can give a precise historical record series file number and identify the immigrant by name or other information.
If the immigrant's date of birth is less than 100 years before the request, the instructions say to attach documentary proof of death, such as an uncertified death certificate, an obituary, a gravestone photo or an individual Social Security Death Index record.
The instructions say not to use Form G-1041A for naturalization records before September 27, 1906, manifest lists before December 1982, deportation records or the return of original documents, which uses Form G-884. Records of living immigrants go through Form G-639.

Yes. The USCIS page lists online filing through its Genealogy Program page, or filing by mail to the Chicago Lockbox.
The instructions list naturalization records before September 27, 1906, manifest lists before December 1982 and deportation records. Records of living immigrants, or of immigrants who arrived after 1945 or naturalized after April 1, 1956, go through Form G-639.
USCIS fees change, so this page does not list one. The USCIS page points to its Fee Schedule, and the instructions point to Form G-1055.
