WT 3: Supplementary Report for Wiretaps Reported in Previous Calendar Years

WT 3, Supplementary Report for Wiretaps Reported in Previous Calendar Years, is a one-page report made in accordance with Title 18, United States Code, Section 2519, and sent to the Administrative Office of the U.S. Courts. The U.S. Courts list it under Other Forms. It is not a form members of the public file: the guidance on the U.S. Courts page says prosecutors use it to record applications reported in prior years that resulted in additional costs, arrests, trials, motions or convictions during the current reporting year.

The form is a table with one row per wiretap: the report year, the A.O. report number, the date of application, and the additional cost, persons arrested, trials completed, motions to suppress and persons convicted. It ends with the name, jurisdiction, address and phone number of the person responsible. The printed form is marked Rev. 10/15, and the page lists it as effective on October 1, 2015.

This page explains the form in plain words and is not legal advice. The office completing it can fill it out free in the browser on this page and download the result without an account.

How to fill out WT 3

  1. 1

    Check for new activity

The guidance says a supplementary report is not required if there has been no additional activity for any wiretap reported in a previous calendar year, and that a separate WT 3 is required for each reporting year with activity.

  2. 2

Find the report year and A.O. number

The guidance says to use the year the wiretap first appeared in the published Wiretap Report and its number there, and points to www.uscourts.gov/Statistics/WiretapReports if either is unknown.

  3. 3

Enter only activity not reported before

Fill in columns 3 to 8. The guidance says to complete all columns for each intercept order, leaving a column blank or entering a zero if no entry is appropriate.

  4. 4

    Use the example row

The form prints an example row for report year 2001, A.O. report number 6, with sample arrests, trials, motions and convictions.
