AO 443, Warrant for the Arrest of a Witness or Material Witness in a Pending Criminal Case, is a United States District Court form from the U.S. Courts. The U.S. Courts page lists it under Law Enforcement, Grand Jury, and Prosecution Forms, effective on November 1, 2011, and the PDF prints the edition AO 443 (Rev. 11/11).
It is a warrant the court issues in a case captioned United States of America v. a defendant. Page 1 is addressed "To: Any authorized law enforcement officer," commands the arrest of a named witness, and is signed by the issuing officer. It ends with a Return the arresting officer completes. Page 2 holds the witness's personal identifiers and is marked Not for Public Disclosure. It is not a form a member of the public completes.
This page explains the form in plain words and is not legal advice. You can fill it out free in your browser on this page and download the result without an account.
