AO 245D: Judgment in a Criminal Case (for Revocation of Probation or Supervised Release)

AO 245D, Judgment in a Criminal Case (for Revocation of Probation or Supervised Release), is the form a United States District Court uses for the judgment on violations of a term of supervision. The U.S. Courts lists it under Criminal Judgment Forms, effective on September 1, 2019, and the edition printed on the form is Rev. 09/19.

The form is a set of sheets: the judgment and the violations, imprisonment, supervised release, probation, criminal monetary penalties and the schedule of payments, plus a Personal Identification Attachment marked Not for Public Disclosure. Sheet 1 and the attachment are always included when printing, and every other sheet has a choice to include it or not.

It is a court document. The judge signs it, the United States Marshal completes the Return, and the defendant signs only the probation office acknowledgment. This page explains the form in plain words and is not legal advice. You can fill it out free in your browser on this page and download the result without an account.
