Tax Information Authorization (Spanish Version)
Form 8821 (sp) is the Spanish version of Form 8821, Tax Information Authorization. The one-page PDF is in Spanish under the title "Autorización para Recibir Información Tributaria" and is dated January 2021 ("enero de 2021", Rev. 1-2021). The IRS made it for taxpayers who read Spanish. The agency name, the labels inside the IRS-use-only box and a few footer items are printed in English.
A taxpayer uses it to name one or more designees who may inspect and/or receive the taxpayer's confidential tax information for the types of tax, form numbers, years or periods and matters listed on line 3. The top of the form says not to use Form 8821 (sp) to request copies of tax returns or to authorize someone to represent the taxpayer, and not to sign it until every applicable line is complete.
The PDF points to www.irs.gov/Form8821sp for its instructions. The instructions cited on this page are the English Instructions for Form 8821. The English edition, Form 8821, has its own page. You can fill out this Spanish PDF free in your browser on this page and download it without an account.

The taxpayer's name and address, taxpayer identification number(s), daytime phone number and a plan number if one applies.
Two blocks for a designee's name and address, CAF number, PTIN, phone and fax, a box to send them copies of notices, and boxes for a changed address, phone or fax.
Four columns: (a) type of tax information, (b) tax form number, (c) years or periods and (d) specific tax matters, plus a box for access through an intermediate service provider.
Line 4 is a box for a specific use not recorded on the Centralized Authorization File (CAF). Line 5 is a box to keep earlier tax information authorizations in place.
The signature, date, printed name and title if any. Two capitalized warnings say an incomplete or unsigned form is returned and a blank form is not to be signed.
Enter the taxpayer's name, address and identification number. The instructions say not to put the designee's details here, and that this address does not change your last known address with the IRS.
Enter each designee's full name, address and CAF number, or NONE if they have no CAF number yet. With more than two designees, check the box and attach a list.
List the type of tax, the form number (the form gives 1040, 941 and 720 as examples), the years or periods and any specific matter. The instructions say a general reference such as all years causes the form to be returned.
Check it only for a specific use the IRS does not record on the CAF. The instructions give disclosures to loan companies or educational institutions as examples. If checked, skip line 5.
The form says the IRS automatically revokes all prior tax information authorizations on file unless you check this box and attach a copy of each one to keep.
Sign, date and print the name, plus a title if signing for an entity. The instructions say mailed or faxed forms need a handwritten signature.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form goes to the IRS. The English instructions list online submission at IRS.gov/Submit8821, which needs a Secure Access account, or fax or mail to the office in their Where To File Chart for where you live.
The chart lists Memphis, TN 38118 (fax 855-214-7519), Ogden, UT 84201 (fax 855-214-7522) and the International CAF Team in Philadelphia, PA 19104 (fax 855-772-3156). It says these numbers may change without notice.
If the line 4 box is checked, the instructions say to mail or fax the form to the IRS office handling the specific matter.
For a purpose other than a tax matter with the IRS, such as income verification for a lender, the instructions say the IRS has to receive the form within 120 days of the signature date.
The instructions say a form with an electronic signature can be submitted only online.
The form says it is returned if it is not completed, signed and dated.

Every line label and note on this PDF is in Spanish. The same authorization in English is Form 8821.
An individual enters their own name, taxpayer identification number and address on line 1. The instructions say a spouse on a joint return files a separate Form 8821 to name a designee.
Line 6 lets a corporate officer, partner, guardian, partnership representative, executor, receiver, administrator or trustee sign, certifying they have legal authority for the matters on line 3.
A designee is any individual, corporation, firm, organization or partnership named on line 2. The designee does not sign the form.
The form says not to use it to authorize someone to represent you or to request copies of your returns. The instructions point to Form 2848 for representation.
The taxpayer named on line 1 completes the form and signs it on line 6. The designees named on line 2 are the people or firms who may then inspect or receive the information. The completed form goes to the IRS.
