Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites (Portuguese Version)
Form 15080 (pt) is the Portuguese version of Form 15080, Consent to Disclose Tax Return Information to VITA/TCE Tax Preparation Sites. The IRS publishes it for taxpayers who read Portuguese and have their return prepared at a VITA or TCE volunteer site. The consent text on the PDF is in Portuguese, under the title "Consentimento para divulgar informações da declaração de imposto aos locais de preparação do VITA/TCE". The header is dated Outubro de 2025 and the footer prints Form 15080 (pt) (Rev. 10-2025).
The form asks whether the taxpayer agrees to Global Carry Forward. The Portuguese text says this lets TaxSlayer LLC, the provider of the VITA/TCE tax software, make the return information available to any volunteer site in the IRS VITA/TCE program that the taxpayer selects for the next return. This edition says the consent is valid until 30 de novembro de 2027 (November 30, 2027).
The English edition, Form 15080, has its own page. You can fill out this Portuguese PDF free in your browser on this page and download the result without an account.

The form says a taxpayer who wants an earlier end date or a narrower scope will deny consent instead of signing.
To consent, the primary taxpayer prints a name, signs and dates the first line.
A secondary taxpayer prints a name, signs and dates the second line.
The Portuguese text says the taxpayer has the right to receive a signed copy of the form.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form is signed at the site that prepares the return. It prints no mailing address.
The form says it is not necessary for using the site's services, and a signature obtained as a condition of those services makes the consent invalid.
The terms say the consent is valid until 30 de novembro de 2027. The federal disclosure adds that a consent with no duration specified is valid for one year from the date of signature.
Consenting allows the preparer to enter a PIN in the tax software on the taxpayer's behalf to verify agreement with the terms.
The form says to contact TIGTA at 1-800-366-4484 if return information was disclosed or used improperly or without authorization.

Two lines, "Nome do contribuinte primário em letra de forma e assinatura" and the same for the "contribuinte secundário", each take a printed name, a signature and a date.
By consenting, the taxpayer allows the return preparer to enter a personal identification number (PIN) in the tax software on the taxpayer's behalf to verify agreement.
The form names TaxSlayer LLC as the VITA/TCE software provider that makes the return information available to the volunteer site selected next year.
The Portuguese text says no consent needs to be given to the VITA/TCE partner preparing this year's return. It helps only if the taxpayer visits another VITA or TCE partner using TaxSlayer next year.
A VITA or TCE site gives the form to the taxpayer, and the taxpayer decides whether to sign it. The Portuguese text says federal law requires that the taxpayer receive this consent form.
