Form 13614-C (pt) is the Portuguese version of Form 13614-C, the Intake/Interview and Quality Review Sheet used by the IRS Volunteer Income Tax Assistance (VITA) and Tax Counseling for the Elderly (TCE) program. The PDF is in Portuguese, titled "Ficha de admissão/entrevista e análise de qualidade", so a Portuguese-speaking taxpayer can answer the intake questions before an IRS-certified volunteer prepares the return. The Portuguese text calls the IRS "Receita Federal".
This edition is dated October 2025 (Outubro de 2025) and its questions refer to 2025. Columns marked for the certified volunteer are completed by the volunteer.
The last page is Form 15080 (pt), a Portuguese consent for Global Carry Forward. The English edition is Form 13614-C. You can fill out this PDF free in your browser on this page and download it without an account.
