VA Form 29-541: Certificate Showing Residence and Heirs of Deceased Veteran or Beneficiary

VA Form 29-541 is the U.S. Department of Veterans Affairs Certificate Showing Residence and Heirs of Deceased Veteran or Beneficiary. The form says it is used to provide information concerning the heirs of a deceased Veteran or beneficiary, and is typically used when an estate is entitled, or payment is made by a set order of individuals defined by law, called order of precedence. VA's form page lists it under life insurance.

The form gives the order of precedence as the surviving spouse, children and descendants of deceased children, parents or their surviving children, the executor or administrator of the estate, and other next of kin under the laws of the Veteran's residence at time of death. Its five pages ask whose estate the payment goes to, the state of residence at death, the spouse, children, grandchildren, parents, siblings and nieces and nephews, then a certification and signature.
