Request for Approval of School Attendance
VA Form 21-674 is the U.S. Department of Veterans Affairs Request for Approval of School Attendance. VA's form page says to use it when claiming benefits for a child who is at least 18 years old, but under 23, and attending school. The page notes that VA automatically removes children from your benefits when they turn 18, so a child who will remain in school past 18 needs their information submitted again.
The PDF has three pages: identification of the Veteran or claimant and the student, school attendance details, the student's income and estate for pension claims, remarks, and the certification and signature, followed by a page of instructions and mailing addresses.
VA's page says you can submit the request online through the Application Request to Add and/or Remove Dependents, VA Form 21-686c. You can fill out the PDF free in your browser on this page and download the result without an account.

The Veteran's or claimant's name, VA file number if applicable and an optional email address.

The student's name, Social Security number, date of birth, whether the student has ever married and the date, and the student's address. The form says to use a separate form for each student.
Whether the student receives Chapter 35, the Fry Scholarship or FECA or attends a wholly federally supported school, continuous attendance, accreditation, term, course start and expected graduation dates, and the last term.
Earnings, annual Social Security, other annuities and all other income, received and expected by calendar year, then savings, securities, real estate other than your home, other assets and total value. The form says not to report VA benefits.
Space for remarks, then the certification, an agreement to notify VA of changes, the required signature in Item 16A, the date, relationship to the student and phone number.
Fill in Sections I and II. If completed by hand, the form asks for ink, neat and legible printing, one letter per box and each applicable circle completely filled in.
The instructions say all claimants complete this part. They say not to report transferred Post-9/11 GI Bill benefits in Item 9A.
For disability Pension or Survivors' Pension only. The form says to complete each income block, write 0.00 where there is no income from a source, and report gross amounts before deductions.
Item 16A is marked required. The certification includes agreeing to notify VA immediately of changes in education, a school transfer, discontinued attendance, receipt of DEA or marriage.
Mail it to the Evidence Intake Center for compensation claims or the Pension Intake Center for pension and survivors benefit claims, or submit online through VA Form 21-686c.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.

The instructions say the form is completed by the person receiving or claiming benefits for a veteran's child who is at least 18 but under 23 and attending school.
The form says the child completes it only after reaching the age of majority and being or becoming entitled to direct payment of VA benefits. It says the age of majority is determined by State law and is 18 in most states.
When the student does not sign, the form says the veteran, surviving spouse, guardian or custodian signs, dates and enters their relationship to the student and phone number in Section VI.
The form says Section IV, the student's income and estate, is completed only if the benefit claimed or received is disability Pension or Survivors' Pension.
