Checklist of Major Requirements for State Home Construction/Acquisition Grants
VA Form 10-0388-14 is the U.S. Department of Veterans Affairs Checklist of Major Requirements for State Home Construction/Acquisition Grants. VA's forms feed lists it under the Veterans Health Administration. Each of its three sections is headed To be filled out by VA, with a date column beside every requirement.
The form is two pages. The header holds the location, description, FAI number, DUNS, date and time submitted and VA cost. Section A covers the initial application phase, with a deadline of April 15 each year for VA receipt. Section B covers the application phase, and Section C covers post grant award items required prior to final payment. The checklist names the other VA forms in the 10-0388 series that go with each phase, and a note says that unless otherwise indicated, documents are scanned and e-mailed to SHCGP.
