DS-5504
Application for a U.S. Passport: Name Change, Data Correction, and Limited Passport Book Replacement
Form DS-5504 is the U.S. Department of State's application for passport holders who can get a new U.S. passport book or card by mail without paying the passport fee again. It covers three cases: a name change by marriage or court order less than one year after a passport issued less than one year ago, identifying information printed incorrectly in your most recent passport, and a passport book limited to two years or less that you paid the full fee for.
If none of those apply, the form says to stop and use Form DS-11 or DS-82 instead. A lost, stolen, damaged or mutilated passport also cannot be replaced with this form; that takes Form DS-11 in person.
You sign DS-5504 yourself and mail it with your current passport. You can fill it out free in your browser on this page and download the result without an account.

Your name as it should appear in the passport, date of birth, sex, place of birth, Social Security number, email, phone, mailing address and other names you have used.
The book or card number and issue date of the passport you are sending in. You sign and date page 1 before moving on.
Page 2 asks for your height, hair and eye color, occupation, employer or school, permanent address, an emergency contact, and planned travel dates and countries.
Whether your name changed by marriage or court order less than one year after the passport was issued, and your new name.
Whether your identifying information was printed incorrectly. You check only the fields to correct, such as name, date of birth, sex or place of birth, and write them as they should appear.
Whether your most recent passport book was limited to two years or less. The book you send in must not be expired.
Fill out both application pages and sign and date page 1. Print each page on a separate piece of paper.
Send the passport book or card with the error, the name to change or the limited validity.
For a name change, a certified marriage certificate, divorce decree or court order. For a correction, a document showing the correct information. For a limited book, evidence of citizenship or identity.
One color photo, 2 x 2 inches, taken in the last six months, on a plain white or off-white background, with no eyeglasses. Staple it at the four corners without bending it.
Mail everything to the National Passport Processing Center address on the form, using the routine or expedited address. The State Department recommends a trackable mailing service.
Track the status at passportstatus.state.gov. Your new passport and returned documents can arrive in separate mailings.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
Routine service goes to National Passport Processing Center, P.O. Box 90107, Philadelphia, PA 19190-0107. Expedited service goes to P.O. Box 90907, Philadelphia, PA 19190-0907.
There is no fee unless you ask for expedited service or 1-2 day delivery for a name change or limited passport replacement. A data or printing error in a valid passport is corrected at no cost. Current amounts are at travel.state.gov/passportfees.
Applicants living outside the United States and Canada cannot use the domestic addresses. Each U.S. embassy and consulate has its own procedure.
An error reported within one year of issue gets a passport valid for 10 years for adults or 5 years for minors. An error reported later gets one valid until the original passport would have expired.
Documents that are not damaged, altered or forged are returned to you. Photocopies are not returned.
Your name changed by marriage or court order less than one year after your passport was issued, and that passport was issued less than one year ago.
Your name, date of birth, sex or place of birth was printed incorrectly, or the passport has a printing error such as missing data, discoloration or crooked printing.
Your most recent passport book was issued at the full fee but limited to two years or less, for a reason other than multiple losses or serious damage.
If your passport was lost, stolen, damaged or mutilated, or none of the three statements fit, the form points to Form DS-11 or DS-82.
The form lists three statements about your most recent passport. If you can answer yes to any one of them, you may use it.
