DS-11
Application for a U.S. Passport
Form DS-11 is the U.S. Department of State's application for a U.S. passport book, a passport card or both, made in person. You present it at an acceptance facility, at a passport agency by appointment, or at a U.S. embassy, consulate or consular agency abroad. The passport is issued to U.S. citizens and non-citizen U.S. nationals.
The form is for people applying for their first U.S. passport, applicants under age 16, and anyone whose previous passport was issued under age 16, issued more than 15 years ago, or lost, stolen or damaged. If none of those apply, the form points to Form DS-82 or DS-5504 instead.
The State Department asks for the form printed on single-sided letter-size paper and not signed until the agent asks. You can fill it out free in your browser on this page and download the result without an account.
