Standard Form 513 (SF 513): Consultation Sheet

Standard Form 513 (SF 513), Consultation Sheet, is a one-page medical record printed as STANDARD FORM 513 (REV. 4-98). Its footer says Prescribed by GSA/ICMR (41 CFR) 101-11.203(b)(10), and GSA hosts a copy in its forms library, where it is listed as Medical Record - Consultation Sheet.

The bottom half is the consultation report, with yes or no boxes for records reviewed, patient examined and telemedicine, an open report area, and a signature, title and date. The last rows identify the patient, the sponsor and the facility.

The top of the form says AUTHORIZED FOR LOCAL REPRODUCTION. You can fill it out free in your browser on this page and download the result without an account.
