Doctor's Orders
Standard Form 508 (SF 508), Doctor's Orders, is a two-sided medical record sheet, Rev. 3/1994. Its footer reads Prescribed by GSA/ICMR, FIRMR (41 CFR) 201-9.202-1. GSA hosts a copy in its forms library.
Each side is a table of orders with a start and stop date and time, an RX column, a Drug Orders column, and columns for the doctor's signature and the nurse's signature. The heading says Sign all orders. The front ends with the patient's identification, a register number and a ward number.
GSA's page says U.S. Government departments, agencies and offices can order printed copies through GSA Global Supply or GSA Advantage. You can fill it out free in your browser on this page and download the result without an account.
