ACH Vendor/Miscellaneous Payment Enrollment
Standard Form 3881 (SF 3881) is the ACH Vendor/Miscellaneous Payment Enrollment Form. Its footer says it is prescribed by the Department of Treasury under 31 U.S.C. 3322 and 31 CFR 210, and it carries OMB No. 1530-0069. The edition is Rev. 2/2003. GSA hosts a copy in its forms library.
The form says it is used for Automated Clearing House (ACH) payments with an addendum record that contains payment-related information processed through the Vendor Express Program. It has three sections, each filled in by a different party: the federal program agency, the payee or company, and the payee's financial institution. The form says recipients of these payments should bring it to their financial institution for completion.
The Privacy Act statement says the Treasury Department uses the information to transmit payment data by electronic means to the vendor's financial institution. You can fill it out free in your browser on this page and download the result without an account.

The instructions say the federal agency prints or types its details and checks the appropriate box for ACH format.
The payee prints or types the name and address that will receive the payments, the Social Security or taxpayer ID number and a contact person.
The form says recipients of these payments should bring this information to the attention of their financial institution when presenting the form for completion.

The form says to make three copies after completing it: copy 1 is the Agency Copy, copy 2 the Payee/Company Copy and copy 3 the Financial Institution Copy.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no mailing address for the completed form. It says to make three copies: one for the agency, one for the payee/company and one for the financial institution.
The burden estimate statement gives an average of 15 minutes per respondent or recordkeeper, depending on individual circumstances.
The Privacy Act statement says the information is required under 31 U.S.C. 3322 and 31 CFR 210, and that failure to provide it may delay or prevent the receipt of payments through the Automated Clearing House Payment System.
Comments on the burden estimate go to the Financial Management Service, Facilities Management Division, Property and Supply Branch, and to the Office of Management and Budget, Paperwork Reduction Project (1530-0069).
