Public Voucher for Purchases and Services Other Than Personal
Standard Form 1034 (SF 1034) is the Public Voucher for Purchases and Services Other Than Personal. The form's header names the Department of the Treasury and cites 1 TFM 4-2000. The edition is Revised October 1987, and the form says Previous edition usable. GSA hosts a copy in its forms library.
The voucher records a payment for articles or services: the U.S. department, bureau or establishment, the payee's name and address, the contract and requisition numbers, and each order with its delivery date, description, quantity, unit price and amount. The lower part, which the form says the payee must NOT use, holds the approval, certification, accounting classification and payment record.
The Privacy Act statement says the information is used to identify the particular creditor and the amounts to be paid. You can fill it out free in your browser on this page and download the result without an account.

Department, bureau or establishment and location, voucher number, date voucher prepared, schedule number, contract number and date, requisition number and date, and Paid By.
Payee's name and address, date invoice received, discount terms and payee's account number.
Shipped from, to, weight and Government B/L number.
Number and date of order, date of delivery or service, a description with the item number of the contract or Federal supply schedule, quantity, unit cost and per, amount and a total. Note 1 says to insert the name of the currency when stated in foreign currency.
Payment type (provisional, complete, partial, final, progress or advance), amount approved, exchange rate, differences, the amount verified box and the certifying officer's signature.
Accounting classification, then the check number, bank, cash amount, date and the payee's receipt, with Per and Title lines.
Fill in the department, bureau or establishment and location, the voucher number and date prepared, and the contract and requisition numbers and dates.
Enter the payee's name and address in the bracketed box, with the payee's account number, the discount terms and the date the invoice was received.
For each order, enter its number and date, delivery date, description, quantity, unit price and amount, then the total. The form says to use continuation sheet(s) if necessary.
The form says the payee must NOT use the space below the total line, where the payment type, approval, certification and accounting classification go.
The approving officer signs over an official title, and the certifying officer certifies the voucher correct and proper for payment. One signature is enough when one person holds both authorities.
Note 3 says that when a voucher is receipted in the name of a company or corporation, the name and capacity of the person signing also appear, for example: John Doe Company, per John Smith, Secretary.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no filing address or submission method. The approval, certification and payment blocks are completed by the government, not the payee.
The articles or services table says to use continuation sheet(s) if necessary. Standard Form 1035 is printed as the continuation sheet for this voucher.
The header cites 1 TFM 4-2000. GSA's page lists 1 TFM 4-2000 and FAR (48 CFR) 53.249(a)(1) as the authority or regulation.
The form says Previous edition usable.
The Privacy Act statement says the information is required under 31 U.S.C. 82b and 82c, for the purpose of disbursing Federal money, and that failure to furnish it will hinder discharge of the payment obligation.

The certifying officer certifies, pursuant to authority vested in them, that the voucher is correct and proper for payment, and adds the date and title.
