Standard Form 329D (SF 329D), Employer Certification, is a two-page certification an employer returns to a creditor agency about a Wage Garnishment Order for one of its employees. The footer says it is prescribed by 31 CFR 285.11, and the form names no issuing agency. GSA hosts a copy in its forms library. The edition printed on the form is STANDARD FORM 329D (REV. 3/2026).
The Notice to Employers at the top says the employer must complete and return the certification to the creditor agency within 20 days of receipt. The creditor agency fills in the first block, with the order dates, tracking number and the employee. The employer completes the rest: when the order arrived, whether the employee still works there, pay for the current pay period and any withholding orders with priority.
