Wage Garnishment Worksheet
Standard Form 329C (SF 329C), Wage Garnishment Worksheet, is a one-page worksheet for figuring the Wage Garnishment Amount to deduct from a debtor's disposable pay. The footer says it is prescribed by 31 CFR 285.11, and the form names no issuing agency. GSA hosts a copy in its forms library. The edition printed on the form is STANDARD FORM 329C (11-98).
Its Notice to Employers says the employer may use a copy of the worksheet each pay period, and that disposable pay includes, but is not limited to, salary, overtime, bonuses, commissions, sick leave and vacation pay. If section 2(a) of the Wage Garnishment Order specifies the dollar amount to be garnished, the form says the employer does not need to complete the worksheet.
Lines 1 to 4 compute disposable pay, and lines 5 to 11 compute the Wage Garnishment Amount as the smallest of line 7, 8 or 10. You can fill it out free in your browser on this page and download the result without an account.

Gross amount paid to the employee, then amounts withheld for federal income tax, F.I.C.A. (social security), Medicare, state tax, city or local tax, health insurance premiums and involuntary retirement or pension plan payments.
Line 3 adds lines a to g as total allowable deductions, and line 4 subtracts line 3 from line 1 to give disposable pay.
25% of disposable pay, the total withheld under other wage withholding orders with priority, and line 6 subtracted from line 5, entering zero if line 6 is more than line 5.
The percentage from section 2(b)(1) of the order times line 4, which the form says may not exceed 15%, an amount equal to 30 times the Federal Minimum wage, and line 9 subtracted from line 4.
The amount on line 7, 8 or 10, whichever amount is the smallest.
If the Wage Garnishment Order already specifies the dollar amount to be garnished, the form says the employer does not need to complete the worksheet.
Fill in the debtor name and Social Security number, then select one pay period frequency.
Enter gross pay on line 1 and each amount withheld on lines 2a to 2g, add them on line 3 and subtract line 3 from line 1 on line 4.
If the wages are not subject to any withholding orders with priority, the form says to skip to line 8. Otherwise complete lines 5 to 7.
Multiply the percentage from section 2(b)(1) of the order by line 4. The form's example: at 15%, multiply .15 by line 4. Take line 9 from its table by pay frequency and subtract it from line 4 on line 10.
Line 11 is line 7, 8 or 10, whichever amount is the smallest. The Notice to Employers says a copy of the worksheet may be used each pay period.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The worksheet prints no filing address and no return instruction. The Notice to Employers presents it as a calculation the employer may use each pay period.
Line 9 is an amount equivalent to 30 times the Federal Minimum wage, printed on this edition as ($5.15). Its table gives line 9 as 154.50 weekly or less, 309.00 every other week, 334.75 2x per month and 669.50 monthly.
The footer prints STANDARD FORM 329C (11-98), Prescribed by 31 CFR 285.11 and AUTHORIZED FOR LOCAL REPRODUCTION. GSA's page lists the same authority and a current revision date of 11/1998. The PDF prints no OMB number or time estimate.
