Standard Form 299 (SF 299): Application for Facilities on Federal Lands and Property

Standard Form 299 (SF 299) is the Application for Transportation, Utility Systems, Telecommunications and Facilities on Federal Lands and Property. It is used to apply for an authorization, which the form lists as an easement, right-of-way, lease, license or permit. This copy is Rev. 10/2023 with OMB Control Number 0596-0249. The form prints no prescribing agency; its public burden statement sends comments to the USDA Forest Service. GSA hosts a copy in its forms library.

The form says applicants should review the whole package, including the instructions, and schedule a pre-application meeting with the agency responsible for processing the application, since each agency may have its own requirements. It asks about the applicant, the project, a map, alternatives, need, and effects on people, the environment and wildlife, with a supplemental page for corporations and partnerships.

The form estimates approximately 8 hours per response. You can fill it out free in your browser on this page and download the result without an account.

How to complete SF 299

  1. 1

    Meet the agency first

The note at the top suggests a pre-application meeting with the processing agency. The form says the application can often be completed at that meeting with the agency representative's help.

  2. 2

    Describe the project in detail

For Item 7, the instructions say to attach preliminary site and facility construction plans. The responsible agency provides instructions when specific plans are required.

  3. 3

    Attach a map

The instructions say the map generally shows the section(s), township(s) and range(s) of the project, with its location as accurate as possible. Some agencies require detailed survey maps.

  4. 4

    Cover alternatives and effects

The instructions ask for only reasonable alternate locations in Item 13, and as much detail as possible in Items 16 to 19, skipping subjects that do not apply, such as a stream the project is not near.

  5. 5

    Complete the supplemental page

Corporations and partnerships check Attached or Filed for each document. The form says to give the file identification when the information is already on file and current with the agency.

  6. 6

Sign and file with each agency

The applicant certifies the information and signs and dates it. The form prints the 18 U.S.C. Section 1001 warning on false statements.
