Standard Form 282 (SF 282): Mandatory Title Check

Standard Form 282 (SF 282), Mandatory Title Check, is prescribed by GSA, FPMR (41 CFR 101-1 1.1304), as printed in its footer, April 1979 edition. GSA hosts it in its forms library. The header says to complete it for each planned production in accordance with the provisions of OMB Circular (A114).

A requesting federal agency fills in its name, the date, an internal control number if required, a contact person, and the intended subject and specific elements of the planned production. The National Audiovisual Center (NAC) completes items 8 through 12 and returns the form, either saying it found no productions that meet the request or enclosing information on productions that may be appropriate, along with a Common Data Base (CDB) search number.

The back reprints an excerpt from the Federal Register, August 1, 1978, on Section 101-11.1304, Centralized audiovisual services. You can fill it out free in your browser on this page and download the result without an account.
