Standard Form 1157 (SF 1157), Claims for Witness Attendance Fees, Travel, and Miscellaneous Expenses, is a two-page claim for a witness's attendance fees, mileage, subsistence and other travel costs in a case. It is a U.S. Department of the Treasury form: the header names the Department of the Treasury and cites 1 TFRM 4-2000, 1157-107. GSA hosts a copy in its forms library. The form has three parts. Part I is an attendance certification with the witness's details, the travel and attendance dates and a signed statement that the witness attended.
