OF1169
U.S. Government Transportation Request
Optional Form 1169 (OF 1169), U.S. Government Transportation Request, is a request for transportation signed by an issuing government officer and tendered to a carrier or agent. Its footer prints OPTIONAL FORM 1169 (REV. 1/2022) and Prescribed by GSA - FMR (41 CFR) Part 102-118, so it is a GSA form, and GSA hosts it in its forms library. GSA's page says GSA's Office of Government-wide Policy is the sponsor of the form and has authorized Federal Departments/Agencies to assign serial numbers and track electronic copies of it internally.
The front names the office to bill, the fiscal data, the issuing government officer, the traveler and any dependents, the carrier, the cities and class of service, excess baggage and special accommodations, with a section for carrier use only. The back prints seven conditions, four instructions and space for the date travel commenced and any service furnished other than requested. The form is marked NONTRANSFERABLE - PENALTY FOR FRAUDULENT OR PRIVATE USE.
You can fill it out free in your browser on this page and download the result without an account.
