Form G-845: Verification Request

Form G-845, Verification Request, is the USCIS form used to verify an applicant's immigration status when the applicant is applying for federal, state, or local public benefits and licenses. The instructions say an agency registered with the USCIS Systematic Alien Verification for Entitlements (SAVE) Program may file it to request immigration status verification information for an applicant.

The benefit applicant does not file this form. The instructions say any agency that has executed a Memorandum of Agreement with the USCIS SAVE Program may file it. Part 1 is for the registered agency only, and Part 2 (USCIS Responses) and Part 3 (USCIS Comments) are for USCIS only.

The USCIS page says to email the SAVE program at [email protected] and receive approval before submitting Form G-845.

How to fill out Form G-845

  1. 1

    Get SAVE approval first

The USCIS page says to email the SAVE program at [email protected] and receive approval before submitting Form G-845.

  2. 2

Fill in the To and From sections

The To section holds the mailing address of the agency's assigned Status Verification Office, listed at uscis.gov/save. The From section holds the agency's name and full return address with ZIP code, which SAVE may show in a No. 10 window envelope.

  3. 3

    Enter the immigration document numbers

The instructions say to enter the A-Number, the Form I-94 number, or both if both appear on the document. The A-Number is the letter A followed by 7, 8 or 9 digits, and the Form I-94 number is 11 digits.

  4. 4

Add the Case Verification Number if SAVE online was used

Enter the 15-digit case verification number if a query was started in the SAVE online system. The instructions say to leave it blank if the online system was not used.

  5. 5

Attach legible front and back copies

Attach copies of the applicant's most recently issued immigration document, front and back. The instructions say a state driver's license, state ID card or Social Security card is not acceptable as other information.

  6. 6

Complete every item in Part 1

The instructions say all items in Part 1 are completed, including Item 19. Type or print in black ink, write N/A where a question does not apply and None for a zero, and keep the applicant's name and Case Verification Number at the top of each page.

Submitting Form G-845, processing and fees

SAVE approval

The USCIS page lists no mailing address for Form G-845. Under Where to File it says to email the SAVE program at [email protected] and receive approval before submitting the form.

Where the request goes

The form is addressed to the USCIS SAVE Program Status Verification Office. The instructions say the address of the agency's assigned office is at uscis.gov/save.

Fee

The USCIS page points to its Fee Schedule page (Form G-1055) for the Form G-845 filing fee, which can change. Check it on the day you file.

Incomplete requests

The instructions say USCIS returns a Form G-845 with no verification response if it is not completely filled out, or if no copy of the applicant's most recently issued immigration document is attached.

When USCIS needs more research

If USCIS selects Item 19 in Part 2, it is searching indices for more information. The instructions say it returns a copy of the unprocessed form with Form G-1120 while the search is pending, and the original once it has a final response.

Edition and signature

USCIS lists edition 08/12/25. A printed copy keeps the edition date and page numbers visible, with all pages from the same edition. The USCIS page says it will reject any unsigned form.

Who files Form G-845?

  • Registered agencies

The instructions say any agency that has executed a Memorandum of Agreement with the USCIS SAVE Program may file this form to request status verification for an applicant.

  • Agency officials

Items 13 to 18 record the registered agency case number and the submitting official's name, title, daytime phone, fax and the date the request was completed.

  • Applicants for benefits and licenses

The applicant is the person whose status is checked. Part 1 lists the benefit sought, such as a driver's license or ID, Medicaid, food stamps, housing assistance, TANF or unemployment insurance.

  • One form per person

The instructions say a separate Form G-845 is completed for each applicant, with copies of documents only for that individual. When a family unit applies for a benefit, each family member gets a separate form.

  • USCIS

Parts 2 and 3 are marked for USCIS only. USCIS records its finding on the applicant's status there, with comments such as a request for a legible or two-sided copy.
