CMS-632FOI
FREEDOM OF INFORMATION ACT REQUEST
Form CMS-632-FOI, titled Freedom of Information Act Request, is a control and tracking form used by the Centers for Medicare & Medicaid Services. It is completed by CMS staff rather than by the person asking for records: the instructions say it is attached to and remains with every Freedom of Information Act (FOIA) request for control and tracking, and that every CMS employee involved in processing the request adds data accounting for that involvement. That data is the base for the Annual Report.
The form records the CMS FOIA request number, key dates, the requester and subject, the category of requester, any program concern about disclosing the records and the actions taken. It then tallies the actual costs of responding, from processing hours to copying and mailing, the fees charged or waived, the people who searched for and compiled the records, and interim reply dates.
The CMS page lists the form as OMB Exempt. You can fill it out free in your browser on this page and download the result without an account.
